OPEN: Direct Data Entry (DDE) Latency Issues and Reason Code 30993

Open May 22, 2025 Status: The Medicare Administrative Contractors (MACs), Common Working File (CWF) Maintainer, Fiscal Intermediary Shared System (FISS) Maintainer and CMS are all researching the cause of the intermittent DDE latency issue and resolve the issue.

Provider Action: Providers that have had claims, hospice NOEs, home health Notice of Admissions (NOAs) or other submission types RTP for reason code 30993 should return for processing. 

Some providers have reported duplication of their submissions with one version processing/approving and the duplicate RTP for 30993. If your submission was duplicated and one processed/approved, you do not need to resubmit the 30993 RTP duplicate. 

Issue Description

Recently, DDE has had intermittent latency issues, which may have caused claims, hospice Notice of Elections (NOEs), home health NOAs or other submission types to return to the provider (RTP) with reason code 30993. The narrative of reason code 30993 is, “A claim has been submitted with a MBI and the MBI/HIC combination was not found on the MBI cache or CWF MBI Crosswalk.”
