Why are there multiple address sections on the 855S and why are they needed?

Below is a list of the sections of the 855S where an address must be reported and why the address is being requested.

Business Location Information
The address must be a specific street as recorded by the United States Postal Service. Post office boxes are not acceptable. 

Important: When completing the business location address, please ensure the address is correct, including the zip code. Any error, including an entry error on the zip code will cause delays in processing.

1099 Mailing Address
Furnish the address where the IRS 1099 form should be mailed.

Correspondence Mailing Address
This is the address where correspondence will be sent to the supplier by the NPWest or another Medicare Administrative Contractor (MAC).

Medical Record Correspondence Address

This is the address where the medical record correspondence from your designated MAC will be sent. This information will be used for any medical record review requests. This address may not be a billing agency, management service organization, or the supplier’s representative.

Revalidation Request Package Mailing Address
This is the address where the NPWest will send the supplier's enrollment revalidation request. Suppliers may also opt in this section to have the revalidation request mailed to the physical location of the business or to the correspondence address.

Remittance Notices/Special Payments Mailing Address
This is the address where all other payment information such as remittance notices and non-routine special payments will be sent by the DME MAC. Suppliers may also opt in this section to have remittance notices and non-routine special payments mailed to the physical location of the business or to the correspondence address.

Medicare Beneficiary Medical Records Storage Address
Furnish the physical address where Medicare beneficiaries’ medical records are stored if the address is different than the business location address. Do not furnish a post office box or drop box. Suppliers may select the same physical address as indicated for the business location.

This information is also required when submitting information via internet-based PECOS.
