This notice serves as a reminder that the deadline for submitting the 2006 Attestation of Data Relating to CMS payment to a Medicare Part D Sponsor is Friday, June 29, 2012. It is HIGHLY RECOMMENDED, that if you have not completed the 2006 Attestation of Data Relating to CMS Payment to a Medicare Part D Sponsor, you do so immediately.
Please be careful to complete the attestation provided in the June 8, 2012 Health Plan Management e-mail titled, “2006 Attestation of Prescription Drug Event Data and Direct and Indirect Remuneration Data for the reopening of the 2006 Part D payment reconciliation”. The attestation is provided in Microsoft Word format as part of the memorandum. Please follow the instructions for completing the attestation, which are provided in the memorandum.
